Primary care is drowning in reactive work. General practitioners (GPs) still manage chronic diseases with outdated tools built for acute care, lacking oversight of their patient populations. They don't know which patients need which care actions when. The result: hours of time wasted on manual follow-up and preventable complications.Cerebro automates chronic care coordination through intelligent remote monitoring and a smart in-house built algorithm. GPs offer eligible patients access to the Cerebro platform, where they track health metrics prescribed by their physician. The Cerebro-algorithm then analyzes incoming patient data, applies evidence-based clinical guidelines, identifies which patients need action, delegates tasks between GPs and nurses, and invites the right patients at the right time. The outcome: GPs manage larger populations more effectively while ensuring quality of care for each patient.We've conducted over 100 interviews with Belgian GPs to validate both problem and solution. Our live pilot is currently running with 7 GPs at a group practice led by Prof. Dr. Bert Vaes, Belgium's leading population health expert. Additionally, a 2-year Master's thesis research project with 4 GPs in training is underway to validate Cerebro's clinical impact.
