Co-Medic offers a powerful solution for structuring medical data. We have built a state-of-the-art data pipeline that recognizes all medical terms in free text (reports, letters, referrals, lab results, ...), structures these terms in FHIR-resources, and then codifies these resources to various international standards.With this data pipeline, we have built several solutions focusing on primary care in Belgium. By closely listening to the needs of our customers, we have builtTools for population health management: a platform for screening, identifying and following up of chronic patients (type 2 diabetes mellitus, heart failure, chronic kidney disease, ...);Tools for processing incoming data: a letter processor that recognizes new medication, diagnoses and care plans from incoming specialist letters;Co-Chat: an AI chat agent that allows primary care professionals to communicate with their patient records (in a GDPR-compliant manner), allowing them to quickly gain relevant insights out of messy EHR data. It is our goal to create solutions with a lasting impact on (primary) care. Our founding team consists of a general practitioner, a pharmacist and a computer scientist.
